Long Term Care
Our long term care team provides a variety of medical services and emotional
support for patients with chronic illnesses and disabilities as well as
for patients recovering from surgery. Some of these services include senior
care, memory care, transitional care and acute rehabilitation.
Senior Care
If your parent, grandparent or aging loved one needs daily medical care
and living assistance, the long term care team at our Senior Care Centers
can help. We provide nursing care, medical services, therapeutic services,
activities and special events, assistance with daily and functional needs,
pet therapy, and housekeeping services in a home-like setting. Our friendly
and compassionate staff makes it their priority to help residents find
a sense of restored purpose and well-being.

Transitional Care
Many patients who undergo major surgery, such as a knee or hip replacement,
need an extended period of time to recover before returning home. You
may not have the necessary medical or assistive equipment or support system
to help care for your needs, such as incision site cleaning, help with
daily tasks and rehabilitation. Our long term care team can help with
this transition, so you can be discharged from the hospital and stay as
a “short-term” resident in our long term care facility until
you feel fit and comfortable to return home.

Acute Rehabilitation
Our long term care team can also help patients who have suffered from an
illness or accident that will impact their daily living, such as a stroke,
debilitating disease, traumatic accident or amputation. Many of these
patients may eventually return to their normal, daily routine after physical,
occupational and speech therapy, and our long term care team can provide
these therapies to help prepare you for returning home.
