Your Medical Records
To obtain a copy of your medical records, please complete the form below
and return it via fax or mail to Southeast Georgia Health System Medical
Records department, along with a copy of a current government-issued photo ID.
Copies of records may also be picked up from the hospital location that
is most convenient for you.
If you need additional information, please contact the Medical Records
department, Monday-Friday, 9 a.m.-5 p.m.

At Southeast Georgia Health System, we offer an online records request
that verifies your identity with a photo of your driver's license.
Simply click on the button below, take a photo via webcam or smartphone,
and follow the steps to submit your request.

Electronic Access to Your Medical Records
We are pleased to announce that you now have 3 ways to electronically access
your Medical Record.

Via the Patient Portal.
Click here.
- Via the Healthelife app. Download the app from the App Store or the Google
Play Store.
- Via an Application Programming Interface (API) of your choice.
Application Programming Interface (API)
We currently have an established connection with the following APIs:
- Apple Health
- Coral Health
- Mylinks.com
For patients wishing to connect to an API, an established connection must
exist between the app you are using, our Health System and our EHR Vendor
(Cerner). Should you choose to connect via an API not listed above, please
have someone from that company contact Cerner at
https://code/cerner/com/. Once the technical specifications have been validated between the app
developer and Cerner, the app developer should then contact the SGHS HIM
Department at 912-466-7159 to request that SGHS connect to the API.
If you do not have electronic portal access, visit the Medical Records
department or your physician’s office to validate your identity
and provide your email address. Once you complete the account set-up and
create a user name and password, you may take advantage of the access
offered by the Healthelife app and/or access via an API.
