Vaccination Information
Thank you for choosing to be vaccinated for COVID-19. Together, we can
and will defeat this disease!
Vaccines are available at two locations:
Brunswick Campus
Southeast Georgia Physician Associates-Primary Care
Outpatient Care Center
2500 Starling Street, Suite 401, Brunswick
Monday-Friday, 2-5 p.m. Walk-ins welcome.
Camden Campus
Southeast Georgia Physician Associates-Primary Care
2060 Dan Proctor Drive, Suite 2100, St. Marys
Friday, 1-5 p.m. Walk-ins welcome.
In addition to the above vaccination locations, you can also visit
vaccines.gov/search to find locations offering the COVID-19 vaccines.
First & Second Dose – Vaccine Eligibility:
Please refer to
CDC Criteria to determine your eligibility. Talk to your pediatrician regarding vaccines
for children 5-11 years of age.

Moderna: Available for people aged 18 and older.

Pfizer: Available for people aged 12 and older.

J&J: Not currently available.
Third Dose – Vaccine Eligibility:

Moderna third doses: Available to those who meet the
CDC criteria issued October 25, 2021.

Pfizer third doses: Available to those who meet the
CDC criteria issued September 24, 2021.
Fourth Dose – Vaccine Eligibility:

Moderna fourth doses: Available to those who meet the
CDC criteria issued March 29, 2022.

Pfizer fourth doses: Available to those who meet the
CDC criteria issued March 29, 2022.
Fifth Dose – Vaccine Eligibility:

Bivalent fifth dose: Available to those who meet the
CDC criteria.
Those who qualify for third, fourth and fifth doses should bring their
vaccine card. To expedite the vaccine process, please download, print,
complete and bring the vaccine
consent form with you to your visit.
Vaccination day
Please bring your Medicare and/or insurance card and a form of identification,
wear a shirt that gives access to the upper arm and wear a mask. Individuals
receiving the vaccine are not personally responsible for the cost of the
COVID-19 vaccine or the administration of that vaccine, but we do charge
your insurance. Following the vaccination, recipients will be required
to remain in an observation area for 15 minutes to ensure they do not
experience any adverse reaction. Recipients receiving the first Pfizer
or Moderna dose must also be available for a second dose 21 or 28 days
later, depending on the type of vaccine received.
Parking in Brunswick: Please be sure to enter the hospital via our Outpatient Care Center entrance.
