Southeast Georgia Health System is committed to enhancing the health and
well-being of the residents in Brantley, Camden, Charlton, Glynn, McIntosh
and Wayne Counties. In keeping with our mission, the Health System continuously
supports the community by reinvesting staff time, efforts and resources
into improving community health.
FY 2022 Community Health Needs Assessment
To better understand the community’s health needs, the Health System
has completed its FY 2022 Community Health Needs Assessment. In addition
to gathering publicly reported data, the Health System sought input from
public health officials, local Federally Qualified Health Centers, local
not-for-profit organizations addressing health needs, community leaders
and physicians. We also conducted citizen surveys, focus groups in six
(6) counties and surveyed physicians in the community. We found that essential
health needs include, among others:
- Collaborate with local mental health providers, emergency medical services
and law enforcement to support appropriate referral systems for patients
with mental health and substance abuse needs. Raise awareness of available
resources within the service area for mental health and substance abuse
treatment through social media and information to referral sources.
- Increase access to primary and specialty care through intensive physician
recruitment in identified areas of need.
- Provide health education and screenings in identified areas of high need
to include obesity/weight management, hypertension, exercise, nutrition/diet,
diabetes, lifestyle education, importance of dental care, and physician
education about opportunities.
- Improve availability of primary care appointments and decrease inappropriate
use of the emergency room.
The Community Health Needs Assessment provides the Health System with a
road map to assist us in determining how we can invest in the community’s
health needs to achieve maximum impact, especially for our most vulnerable
citizens. It also provides a welcome opportunity to work collaboratively
with key partners in our community to make Southeast Georgia a healthier
place to live. We are excited to continue to invest in the community’s
health and look forward to implementing solutions to gaps in care identified
